Asthma Action Plan

An asthma action plan is a plan that is used for children who suffer from asthma.  It is a form that you, as the health care provider, complete telling your patient how to manage their asthma:

  • There are two action plans you can use. You can find both options here (1 and 2).
  • Once, a patient prints the form and gives to you, complete it and return it to the patient.
  • Your patient will keep a copy and use it as a guide for managing their asthma.
  • Your patient will also give a copy to the school nurse or health aide so your patient's asthma can be managed at school.
  • Note: everyone's asthma is different and an asthma action plan should always be developed by a medical provider with a patient's individual health history in mind.
